The Center for Learning, Access, and Student Success is responsible for (a) securing the disability documentation and information disclosed by a student seeking accommodations and (b) maintaining this information in a confidential record system separate from the student’s academic file.

Need to Know Basis

Disability information is shared with faculty and staff on a limited “need to know” basis.  Common situations for this need include implementing testing accommodations or arranging accessible housing. A general Authorization for Release of Information is requested before sharing disability related information with groups outside of Wake Forest. 

Disposal of Records

Confidential records are maintained for seven years after the student has graduated or left Wake Forest University. Paper files are shredded after seven years and electronic files are archived after graduation.
