• Designated Office

The Center for Learning, Access, and Student Success is responsible for (a) securing the disability documentation and information disclosed by a student seeking accommodations and (b) maintaining this information in confidential locked files separate from the student’s academic file.

  • Need to Know Basis

Disability information is shared with faculty and staff on a limited “need to know” basis.  Common situations for this need include implementing testing accommodations or arranging accessible housing. A signed Release of Information is requested before sharing information with professors each semester. A general Authorization for Release of Information is requested before sharing disability related information to parents as applicable.

  • Disposal of Records

The confidential file is maintained for seven years after the student has graduated or left Wake Forest University. The file is then shredded.
